Parent Payment Agreement Parent Payment AgreementPolicy #1 I understand my tuition will not be discounted for any holidays Kid Academy is closed, nor will it be discounted any days Kid Academy must close due to State Regulation Training or severe weather conditions/natural disasters/pandemics. (Example: Hurricanes) Yes I agreePolicy #2 I understand my tuition will not be discounted for any reason for any days my child may not attend Kid Academy including family emergencies, illness, pandemics, COVID-19, vacations, or suspensions. Yes I agreePolicy #3 I agree to pay my weekly balance by 6:00pm each Friday (upcoming weekly tuition charges & any past balances on my account). I understand if my account has a balance after 6:00pm each Friday my account will be charged a late fee of $19.00, and if my account still has a balance each Monday at 6:00pm, I understand my account will be charged an additional late fee of $36.00. I understand that each Tuesday, Wednesday, and Thursday that my account has a balance, I will be charged an additional late fee of $7.00 each day. I understand if I have a past due balance on Monday at 6:00pm I will not be able to bring my child back to Kid Academy until my balance is paid in full unless approved by management. I do fully understand and agree my balance is still due even if my child is not at Kid Academy on payment due dates. I agree to pay the Late Pick-Up Fee of $1.00 per minute per student after 6:00pm until my child is picked up. The late pick-up fee must be paid before your child returns to Kid Academy. Yes I agreePolicy #4 I understand that I will have to choose one of the following three options: Summer only, a 10-week plan, or a full one-year plan. I fully understand these options, and the Parent Commitment Agreement I have signed. Yes I agreePolicy #5 I agree if I withdraw my child from Kid Academy for any reason, I agree to give a ten-week written withdrawal notice. I agree to policy number four of this agreement and understand my account could be turned over to Justice Court and /or a collection agency of Kid Academy's choosing after two weeks of unpaid tuition fees continue to accrue. At turnover, I agree to the following charges #1) my balance the last day my child attended Kid Academy. #2) six weeks of tuition to cover the ten-week written withdrawal notice, #3) any and all weekly late fees, #4) a $1,000.00 penalty fee for not fulfilling my ten-week written withdrawal notice, #5) attorney fees, #6) all court costs, #7) any collection agency fees, and #8) any other additional cost accrued while collecting the past due debt.
